HEALTH CONNECT DEMO USER AGREEMENT (Version 1.2.1)

This agreement applies to the Health Connect DEMO environment only. By clicking "I Agree & Continue", you agree to the terms below.

1. Purpose & Scope

Health Connect provides tools to document pet care, coordination workflows, and related records. Health Connect does not determine legal custody, placement, or medical decisions.

2. Privacy & Data Handling

Health Connect is not a HIPAA-covered entity. However, the platform follows privacy and data-handling practices inspired by HIPAA principles as guidelines for safeguarding sensitive health-related information.

Data is private by default and shared only with authorized parties involved in a pet's care, such as veterinarians, facilities, rescues, foster caregivers, and transportation providers when applicable.

Demo rule: Demo mode uses sample data. Do not enter real personal information, payment data, or real medical information.

3. Data Sharing & Authorized Access

Health Connect does not sell or publicly disclose data. Access is granted only through defined roles, workflows, and user authorization.

4. Platform Evolution & Future Features

Health Connect may add, modify, or retire features over time. New features that materially affect data use or sharing will require updated disclosures or user acknowledgment.

5. Demo Mode

Demo mode is provided for evaluation and uses sample data. Demo data may be reset, refreshed, or deleted at any time.

6. Cookies / Local Storage (Demo Only)

This demo uses essential cookies and/or local storage to keep the portal working properly and to remember that you accepted this agreement, so you only have to accept it once per individual user on this device/browser.

No marketing cookies are required for the demo.

7. Limitation of Responsibility

Health Connect facilitates documentation and coordination only and does not guarantee outcomes or placements. The service is provided "as-is" for evaluation purposes.
